Job description
- Basic Admin duties
- Provide care / case management service to clients
- To assist in arranging for Pre-admission meetings
- Participates in multi-disciplinary discussion for the management of clients.
- Collating application documents to follow up with the referral agency on the submission of supportive documents * Checking center vacancies
- Arranging orientation for approved cases- to assist in contact new applicant to arrange orientation date
- Admission Undertaking-to assist in preparing the undertaking form for new applicant
- Assist in documenting the rejected/withdrawn case
- Assist in preparing the Medical / Transport subsidies application for patient
- Ensure a high standard of professional services to customers
Job Requirements:
- Singaporean Only
- Min Diploma/ Degree in Social Science/Sociology or equivalent
- Experience in Community Care / Dialysis setting is of an added advantage
- Good interpersonal skills and possess good customer service skills
- 5.5 days work week
