- Plan and deliver physiotherapy rehabilitation services at the Senior Care Centre in collaboration with the Centre Manager and Occupational Therapist.
- Conduct initial assessments for clients upon admission and perform periodic reassessments when required.
- Develop and implement appropriate and effective physiotherapy treatment plans tailored to clients’ needs.
- Participate in multidisciplinary case discussions with the SCC care team.
- Contribute to the development and ongoing review of clients’ Individual Care Plans (ICP) to ensure comprehensive and holistic care.
- Organize and facilitate group therapy sessions and activities for day care clients.
- Review and update programme schedules when necessary to enhance the quality and effectiveness of therapy services.
- Ensure accurate and timely documentation of all therapy-related records
Job Requirements
- Degree/Diploma in Physiotherapy
- Full registration with APHC
- Minimum 3 years of working experience in hospital or relevant setting
- Working hours : 5 days work week
