- Design personalised care plans tailored to the specific needs and goals of each resident.
- Implementation & Monitoring the outlined care interventions and conduct regular evaluations to assess progress and effectiveness.
- Create and facilitate structured programmes and activities that are aligned with the residents’ therapeutic and well-being objectives.
- Prepare and submit comprehensive six-monthly review reports, alongside maintaining monthly interview records for a caseload.
Job Requirement :
- Min Diploma/Degree in Social Work
- At least 1 year of relevant healthcare industry experience
- Working Hours : Monday to Thursday : 8.00am to 5.45pm, Friday: 8.00am to 4.45pm
