- Plan and deliver occupational rehabilitation therapy services at the Senior Care Centre in collaboration with the Centre Manager and Physiotherapist.
- Perform initial assessments for clients upon admission and conduct periodic reassessments when required.
- Develop and implement suitable and effective therapy interventions tailored to each client.
- Participate in multidisciplinary case discussions with members of the SCC care team.
- Assist in developing and monitoring clients’ Individual Care Plans (ICP) to ensure comprehensive care and well-being.
- Design and facilitate group therapy programmes and activities for day care clients.
- Review and adjust programme schedules when needed to improve the quality and effectiveness of therapy services.
- Ensure accurate and proper documentation of all therapy records.
Job Requirements and Qualifications:
- Bachelor degree / Diploma in Occupational Therapy
- Full registration with APHC
- Minimum 3 years of working experience in hospital or relevant setting
- Working hours : 5 days work week
