Manage referrals and coordinate admission for residents of a nursing home

Collaborate with a multi-disciplinary team of Doctors, Nurses, Medical Social Workers and other Allied Health Workers from other healthcare institutions to ensure the smooth transition.

Job Requirement:

  • Singaporean Only
  • Minimum 1 year of relevant experience in customer service/ social service/ welfare work will be preferred.
  • Professional qualifications in Business Admin, Social Work, Nursing or Allied Health disciplines or equivalent.
  • Strong writing, communication and presentation skills
  • Able to work independently
  • Proficient in Microsoft Office Word, Excel and Power Point
  • Office hour
