Job Scope:
- Facilitate structured and empathetic Advance Care Planning conversations with patients and their families.
- Educate individuals on goals-of-care, advance directives, and other end-of-life care options.
- Work collaboratively with an interdisciplinary healthcare team to ensure ACP documents are accurately integrated into patient records and effectively communicated.
- Actively contribute to the development and rollout of ACP programs and initiatives within the organization.
- Uphold the highest standards of confidentiality, ethics, and organizational policy in all interactions.
Job Requirements:
- Min degree in Social Work/Counseling or relevant
- Holds an ACP Certificate
- Working hours: Mon to Fri, 9am to 6pm (open to work on Weekends)
