SOCIAL WORKER

Job Description

• • Evaluate and Assess specific care requirements of individual elderly and develop care plans tailored to their needs.
  • Create and execute programs for elderly utilising data analysis to monitor the effectiveness and adjust according to the needs.
  • Collaborate with various stakeholders to coordinate care, facilitate service referrals, activity participate in case discussions to ensure comprehensive support.
  • Implement and evaluate new activities, review and initiate area for enhancement.
  • Mentor volunteers, manage budgets and resources, and collaborate with other organizations to deliver programs effectively.

Job Requirements:

• Diploma/Bachelor’s Degree in Social Work.
• Preferably relevant experience in Social Service sector, working with elderly clients or any related field.
• Proficient in Microsoft Office (MS Word, Excel and PowerPoint).
• Able to work independently and in a multidisciplinary team to meet deadlines.
• Good interpersonal and communication (oral and written skills).
• Possess some knowledge of programme design, implementation and evaluation.
• Willing to work Mon to Fri 9am – 6pm.
