• Implement, execute care planning for resident
• Conduct comprehensive assessment of each resident’s physical, emotional social and cognitive needs.
• Collaborate with the resident, family and healthcare team to set achievable goals.
• Tailor interventions based on personal needs
• Coordinate care and liaise with stakeholders
Job Requirements:
• Degree in Social Work, Psychology or Counselling, or a Graduate Diploma in Social Work, Psychology or Counselling.
• Willing to comply with the Standard Operating Procedure when discharging duties.
• At least 1 year of relevant experience.
