- Conduct holistic assessments of children and families to identify their needs, strengths, risks and areas requiring support.
- Engage and collaborate with parents to explain assessment outcomes, programme recommendations and appropriate follow-up support.
- Develop and coordinate intervention and care plans with multidisciplinary teams, families and community partners.
- Provide social work input and professional recommendations to support strengths-based and child-centred intervention planning.
- Identify and facilitate referrals to relevant internal and external community resources and services.
- Work closely with community agencies to ensure coordinated and holistic support for children and families.
- Maintain accurate and timely case documentation, reports, records and administrative submissions, including relevant government applications.
- Participate in case discussions and multidisciplinary meetings, contributing social work perspectives and maintaining effective case records.
- Monitor and coordinate follow-up actions to ensure continuity and effectiveness of support provided.
- Build and maintain positive professional relationships with parents, preschools, community agencies and other stakeholders.
- Support collaborative interventions addressing children’s developmental, behavioural, social and emotional needs.
- Stay informed about available community resources and services to enhance referral and support options for families.
· Job Requirements:
- Min Degree in Social Worker
- Min 1 year experience working with special needs families
- Willing to relocate to Singapore
