- Assist the Social Worker in engaging parents and families to understand their child’s needs, programme recommendations and available support.
- Support the coordination of appointments, meetings, follow-ups and case-related activities with families and professionals.
- Assist in gathering relevant information for assessments and case reviews under the guidance of the Social Worker.
- Support referrals to appropriate internal, community and external services based on identified needs.
- Maintain accurate and up-to-date case records, documentation and administrative records.
- Assist with preparing case notes, reports, forms and relevant submissions in a timely manner.
- Participate in case discussions, team meetings and multidisciplinary meetings where required.
- Support the Social Worker in coordinating care plans and follow-up actions with families, preschools and community agencies.
- Maintain regular communication with parents and relevant stakeholders to facilitate smooth service delivery and follow-up.
- Assist in organising and coordinating information from community resources and service providers.
- Support the implementation of intervention and support plans addressing children’s developmental, behavioural, social and emotional needs.
- Establish and maintain positive working relationships with families, colleagues, preschools and community partners.
- Maintain confidentiality and uphold professional standards when handling information relating to children and families.
· Job Requirements:
- Min Degree in Social Worker
- Min 1 year experience working with special needs families
- Willing to relocate to Singapore
